The "octo" is your safe second, to be handed off to another diver in an emergency.
These are usually detuned regulators, so they have a smaller chance at free flowing at depth.
Many are the same regulator as a normal second stage regulator, just with the detuning above. Some are specifically designed to be an octo.
Just to clarify, you first stage regulator is attached to your tank. Your second stage regulator is your primary breathing regulator. The octo is also called the "safe second" stage, to be used in an emergency.
